The use of aldosterone blocking agents (spirolactones) for natriuresis in ascitic patients has been well demonstrated. Since low sodium and high potassium intakes stimulate aldosterone production the effect of variations in electrolyte intake upon the natriuretic effect of Aldactone (spironolactone) has been studied. In addition, by balance technics, the effect of this agent and of ascitic fluid mobilization upon nitrogen and electrolyte metabolism has been determined.
In four patients given spironolactone (400 mg. daily) and low sodium diets (10 to 20 mEq.), raising potassium intake from 80 mEq. daily to 160 mEq. increased sodium excretion by 50% or more with
