To the editor: In a recent perspective, Robin (1) criticizes the overuse and abuse of pulmonary arterial catheters. We share his concern about the indiscriminant use, the many risks, and the potential for inaccurate and therefore misleading data. However, we question his recommendation to curb drastically the use of these catheters until further trials are done.
Robin finds little evidence in the literature supporting the benefits of pulmonary flow catheters. He cites an editorial by Lowenstein and Teplick (2) as notable for suggesting the need for prospective clinical trials, but ignores its message regarding ischemia detection and treatment with vasodilators.
