One of the foremost frustrations of physicians today is the escalating bureaucratization and manipulation of health care delivery. Although increasing pressure by third-party payers and health maintenance organizations to control medicine is problematic, physicians' major nemesis has been state-sponsored Medicare and Medicaid programs. The government's threats of mandatory assignment, freezes on reimbursement, and voluminous paperwork have infuriated many caregivers. Despite the importance of Medicare to the health care system, few people understand the historical creation and ongoing evolution of the Medicare program.
