The purpose of this paper is to present observations we have made which provide a physiologic basis for the use of positive pressure respiration in the treatment of acute pulmonary edema. For the most part positive pressure has been thought of as a method of resuscitation such as that accomplished by the pulmotor in accidental asphyxia. The function of pressure in the respired air has, however, a broad physiological significance, being employed by the human organism itself as a compensatory mechanism as well as lending itself to therapeutic application in inhalational therapy.
