Moderate enlargement of the left atrium in mitral stenosis is not an unusual finding, but massive dilatation is considered a rare entity. Since Owen and Fenton1 described the first case of massive dilatation in 1901, fewer than 40 cases have been reported.
The following case history is presented because it illustrates the progressive enlargement of the left atrium resulting in massive dilatation. The course of the disease was observed with x-rays and physical examinations over an interval of 12 years.
