In no field of biology and medicine is confusion more rampant and conceptual clarity more difficult to attain than in the field of acid-base equilibrium. This has been due to a variety of confusions: confusion between various terminologies and concepts, confusion between the concentration of free ionized hydrogen and the amounts of total potentially ionizable hydrogen in the buffer systems of the body, confusion between the events of transient and of steady states. In addition, some questions remain concerning the relative roles of the several intrarenal processes involved in the defective excretion of hydrogen ion in various types of diseased
