The past decade has brought increasing recognition of the role played by peripheral transport mechanisms in the regulation of thyroid hormone economy. Electrophoretic studies of serum have revealed the presence of 2 proteins, thyroxine-binding globulin (TBG) and thyroxine-binding pre-albumin (TBPA), which together bind all but a small fraction of the thyroxine in the blood. Protein-bound thyroxine is in equilibrium with a very low concentration of "free" or unbound thyroxine. The concentration of the latter varies inversely with the binding activities of TBG and TBPA and directly with the total concentration of hormone in the blood.
