In infection by herpes zoster there is an acute inflammation not only of the root ganglion, but also of the peripheral portion of the nerve. Histological evidence derived from light and electron microscopic studies of biopsy material will be presented. Similar studies on material from patients with post-herpetic neuralgia show dense fibrosis of the peripheral nerve and much collagen material. The presence of this scarring suggested that high doses of steroids might reduce the inflammatory reaction in the acute stage and might therefore reduce the fibrosis, which is thought to be the cause of post-herpetic neuralgia.
