Portal-systemic collateral circulation develops gradually after any occurrence that gives rise to increased pressure in the portal vein. Cirrhosis of the liver giving rise to increased resistance to the passage of blood through the hepatic sinusoids is the most frequent cause. Similarly, any factor compromising the outflow of blood from the hepatic veins will cause the development of a portal-systemic collateral circulation, as will direct encroachment on the portal vein. Except for hemorrhage from gastroesophageal varices, no great harm seems to be done by the diversion of portal blood to collateral channels and the systemic circulation.
