Clinical hypotension and shock in the absence of blood loss often is characterized by low or normal right atrial pressure and low cardiac output. A rapid infusion of 500 ml dextran in 19 patients with this hemodynamic pattern resulted in marked clinical and circulatory improvement. Arterial pressure rose from an average of 80/45 to 113/59 mm Hg; cardiac output increased from an average of 3.1 to 5.8 liters/min; and peripheral vascular resistance fell from a mean of 1,637 to 1,035 dynes sec/cm5.
