Ten patients given tritium-labeled digoxin for congestive heart failure subsequently died from 3½ hours to 6 weeks after receiving the medication. Postmortem examination was done and samples of heart, liver, kidney, adrenals, thyroid, lung, brain, pancreas, spleen, skeletal muscle, and diaphragm were obtained. The tissues were homogenized, lyophilized, and extracted with chloroform. The extract was passed through an alumina column and eluted with a chloroform-ethanol mixture. This method will recover 96% of digoxin, its metabolites or both. The highest concentration of digoxin per gram of wet tissue was present in heart muscle in patients with an elevated blood urea nitrogen
