To the Editor: In the article "Valve Replacement During Pregnancy" (Ann. Intern. Med. 67: 1032, 1967) the authors point out the risk to the fetus attendant on the use of anticoagulants of the coumarin family. I wondered why, in the course of treatment of the patient under discussion, heparin was not substituted for warfarin till the twenty-eighth week of gestation. Was this due to an oversight or was this course of therapy chosen because of a rationale that I do not know? It would seem to me that, if heparin is preferred over the coumarin derivatives because of its higher
