It is commonly accepted that plasma renin concentration and plasma renin activity is low in patients with primary hyperaldosteronism although the literature includes relatively few cases studied in this way. We report here the results of an analysis of plasma renin concentration in 34 further surgically confirmed cases of this syndrome. In most instances, plasma renin concentration fell below or close to the lower end of the normal range. Plasma renin concentration may increase to values in the upper part of the normal range during acute dietary sodium deprivation and to higher levels during more prolonged treatment with spironolactone.
