To the Editor: Recently in the ANNALS (1) we reported the diagnosis and treatment of Pneumocystis carinii pneumonia in patients undergoing chemotherapy for malignancies. Two of the seven patients were treated with pentamidine isethionate with only a presumptive antemortem diagnosis; the diagnosis in each case, however, was confirmed at autopsy. Neither significant response to therapy nor toxicity was noted. We stressed the importance of pretreatment lung biopsy in evaluating effectiveness of this drug, since several other patients had been treated because of a similar presentation with recovery but without biopsy evidence of P. carinii pneumonia.
