If an individual sets out to write a unique text, he might not choose the topic of ischemic heart disease. The published books treating this subject could fill many shelves of a library. The present authors, however, have managed to do more than merely add one more book to the burgeoning literature. This text is full of insights derived from a lifetime of intimate involvement with ischemic heart disease.
