To more accurately determine the value of the electrocardiogram (ECG) in diagnosing coronary artery disease, 48 patients with a history of chest pain and ECG evidence of diaphragmatic myocardial infarction (Q ≥ 0.02 sec in 2 of the leads II, III, or aVF), and complete cardiac catheterization studies including coronary arteriography and left ventricular angiograms were studied. All patients who showed evidence of anterior myocardial infarction (12) or true posterior myocardial infarction (4) in addition to diaphragmatic myocardial infarction were found to have significant disease of all three main coronary arteries.
