Patients seen during a 15-year period at the North Carolina Memorial Hospital, Chapel Hill, N. C., were reviewed to determine the frequency of transitions among polycythemia vera, myelofibrosis with myeloid metaplasia, and chronic myelogenous leukemia. This was supplemented with a review of all available literature purporting to show such transitions. Polycythemia vera appears to transform into myelofibrosis with myeloid metaplasia, but whether this latter illness is the same disorder as the idiopathic form is uncertain. No other transitions were conclusively demonstrated. Evidence of such transitions is not adequate to support the concept that these diseases represent merely different parts of a spectrum of a pathologic state.
