The effectiveness of bretylium tosylate (5 mg/kg) as an antiarrhythmic agent was studied in 10 digitalis-sensitized and normal intact dog hearts. With a pacemaker catheter AV node and His bundle potentials were recorded with and without atrial pacing. Atrioventricular conduction, intraventricular conduction, heart rate, blood pressure, and ventricular automaticity were investigated. The ventricular escape time after vagal-induced asystole defined ventricular automaticity. Digitalis toxicity was produced with acetylstrophanthidin before and after bretylium. With atrial pacing, bretylium increased atrioventricular conduction 33% and acetylstrophanthidin increased it 24%; however, bretylium and acetylstrophanthidin's effect was 119%.
