Sulfonylureas are believed to act by stimulating insulin secretion and are presumably ineffective in more severe diabetics. We investigated insulin levels in response to intravenous glucose before and after treatment with chlorpropamide in maturity-onset diabetics with fasting blood glucose levels of from 74 to 420 mg/100 ml. The hypoglycemic effect of chlorpropamide was greater in those with higher initial fasting blood glucose. After 1 week's treatment the fasting blood glucose decreased an average of 52 mg/100 ml or 23% in those with initial values above 130 mg/100 ml, and 17 mg/100 ml or 14% in those below this level. The
