Chlorpropamide has been shown to produce an antidiuresis when administered to patients with antidiuretic hormone (ADH)-sensitive diabetes insipidus (DI). The mechanism by which the drug acts has been studied both in patients with DI and in rats with hereditary hypothalamic DI and with a complete deficiency of ADH. Fourteen patients with various degrees of severity of DI and nine normal water loaded subjects were studied under metabolic conditions on a fixed sodium and potassium intake. By means of a dehydration test, 11 of the patients were shown to have some degree of residual ADH function while 3 patients were considered
