The cardinal immunological defect in sarcoidosis is depression of delayed-type hypersensitivity which is readily demonstrated by skin tests using tuberculin, Candida albicans, or the chemical dinitrochlorobenzene (DNCB). It is also known that in vitro culture of lymphocytes from sarcoidosis patients fail to undergo blastic transformation in response to phytohemagglutinin (PHA).
Induction of in vivo dinitrochlorobenzene sensitisation has been correlated with in vitro cultured lymphocyte transformation in 21 patients with histologically confirmed sarcoidosis and an equal number of matched control subjects. Failure to sensitise with DNCB was noted in 71% of sarcoidosis patients and 14% of controls. Poor lymphocyte transformation occurred
