To the editor: The recent article by Dr. Macklem and colleagues entitled "Chronic Obstructive Disease of Small Airways" (Ann Intern Med 74:167-177, 1971) describes a group of patients who are morphologically heterogenous but clinically and pathophysiologically similar. The cause of such pulmonary disease is frequently unclear, and, more often than not, the therapeutic maneuvers used are of little benefit. Occasionally, however, as in Case 5, results with steroids are dramatically successful. A simple but unfortunately neglected technique that may allow early recognition of patients such as the one in Case 5 is the examination of the sputum for presence of
