Catecholamine inhibition of insulin secretion contributes to glucose intolerance in patients with pheochromocytoma. Serotonin inhibition of insulin secretion in animal models suggested that glucose intolerance may occur in the carcinoid syndrome. A 60-year-old man presented with metastatic bronchial carcinoid tumor, glucose intolerance, and impaired insulin secretion. There was no family history of diabetes. Streptozotocin, effective in metastatic insulinomas, was given in monthly injections of 2 g for 5 months beginning immediately after control studies were done. There was no dietary alteration. After 2 months of therapy, intractable diarrhea and flushing attacks ceased; weight increased 10 lb; urinary 5-hydroxyindoleacetic acid (5-HIAA)
