The primary function of the lower esophageal sphincter (LES) is to prevent the reflux of gastric contents into the esophagus. Recent studies have shown that symptomatic gastroesophageal reflux correlates well with resting sphincter pressure below 10 mm Hg. Anticholinergic preparations have been shown to adversely affect the lower esophageal sphincter, causing decreased sphincter pressure and increased gastroesophageal reflux. We have studied the effect of the cholinergic agent bethanechol on sphincter pressure in normals and in patients with symptomatic gastroesophageal reflux caused by lower esophageal sphincter incompetence.
