Potassium has been used as an antiarrhythmic agent, but its value has been thought to be limited by its prolongation of intraventricular conduction. To investigate the effects of hyperkalemia on the ventricular specialized conduction system, isotonic potassium chloride was infused into intact anesthetized dogs. Fluoroscopically placed electrode catheters recorded His bundle and left anterior false-tendon electrograms simultaneously with the peripheral electrocardiogram. Arterial and coronary sinus potassium (K+) were sequentially monitored. Delays in all segments of conduction pathways (His-false tendon, false tendon-Q, QRS) were progressive and related directly to potassium increments.
