There is considerable doubt as to the value of the ECG and vectorcardiogram (VCG) in the diagnosis of myocardial infarction in the paced heart because pacemaker rhythm causes distortion of the normal wave forms. The ECG and VCG in patients with right ventricular pacemakers demonstrate QRS-T alterations resembling those of left bundle branch block (LBBB), an entity known to conceal the presence of infarction.
