To the editor: In their report, "A Radial Steal Syndrome with Arteriovenous Fistula for Hemodialysis" Ann Intern Med 75:387-394, 1971), J. A. Bussell, J. A. Abbott, and R. C. Lim put forward the hypothesis that steal of radial bloodflow to the fingers is a common occurrence and may be disabling in patients with side-to-side radial-cephalic arteriovenous anastomosis for chronic hemodialysis. The evidence from their very elaborate studies looks convincing, and their proposal to create preferentially end-to-side arteriovenous fistulas appears logical. Nevertheless, we should like to propose a different therapeutic approach that relies on the tenuous ground of a single observation.
