Medical education in the United States is undergoing marked changes. Curricula are being shortened, the time required for residency training is being decreased, the internship is being phased out, and track systems leading to early medical specialization are being developed. The organizations responsible for medical education (medical schools, hospitals, specialty boards, and medical associations) appear to be acting independently in modifying undergraduate and graduate training, without adequate dialogue among concerned groups. This fragmentation in the educational system may shortly precipitate a crisis in medical education of a severity comparable to that in the provision of health care.
