To the editor: In the April 1973 issue (p. 580), in the excerpt "'Cost/Benefit' in Medicine" from The Medical Journal of Australia, I was particularly interested in the last sentence, "Is the benefit worth this cost to the community?" This question directly points up a dilemma that must constantly be faced by those of us who practice under comprehensive government-financed medical care and hospital care insurance schemes. In our decision-making we must constantly ask ourselves the question, "To whom are we responsible, the insurance agency or the patient?"
