The problem-oriented medical record is more than a format for recording clinical data: it is a logical, if not essential, approach to the practice of medicine. Having heard lectures by Dr. Lawrence Weed and having read about problem-oriented records, clinicians feel inspired to adapt this newly found wisdom to their care of patients. Then the difficulties begin. What a critique it is of the traditional approach to the practice of medicine that problem-oriented patient care should seem so alien.
Thus a book with the promising title Implementing the Problem-Oriented Medical Record creates great expectations. Unfortunately it does not fulfill this
