Medical oncology as a therapeutic discipline began in the late 1940s with the introduction of the nitrogen mustards and antifolate agents. In the subsequent 25 years we have witnessed the bewildering development of a multitude of chemotherapeutic agents characterized, in general, by a low therapeutic index. The use of these agents, singly and in combination, has provided an unprecedented challenge to the pharmacologist and the chemotherapist.
