To the editor: Asmundsson and Kilburn (Ann Intern Med 80:54-57, 1974) are to be congratulated and encouraged for their attempts to define those clinical, pathological, and physiological factors affecting the survival of patients after acute respiratory failure. But the overall results, as plotted in their Figure 1, seem to me susceptible of an explanation they have not mentioned. Their figure shows that about half the patients died within 6 months after admission to the Durham Veterans Administration Hospital. This experience seems to me typical of many other reports.
