To the editor: I find it interesting that Sackett and colleagues made the analogy between their study (The Burlington Trial of The Nurse Practitioner: Health Outcomes of Patients (Ann Intern Med 80:137-142, 1974) and a trial of a pharmacologic agent. I am quite certain that if they had indeed been testing a drug, major methodological changes would have been required for publication. Several defects should be noted.
In their assessment of physical function, the authors do not include any indexes that would reflect mild or even moderate decline in function of the major organ systems, which ordinarily would not be
