To the editor: McCloskey and colleagues reported in the December 1974 issue (Ann Intern Med 81:788-791, 1974) their experience in the treatment of a large number of Corynebacterium diphtheriae carriers. Although I agree with the need to treat carriers and the recommendation that erythromycin be the preferred antibiotic, I feel it is important to mention several problems related to the C. diphtheriae carrier state and the evaluation of its treatment.
McCloskey and associates stated that treatment of diphtheria carriers is superior to no treatment because one Russian report showed that only 12% of carriers become C. diphtheriae-negative when followed for
