To the editor: The paper by Dowling and associates (1) and subsequent correspondence (2, 3) indicates to us a need for further discussion of the problems associated with bacterial endocarditis in the Marfan syndrome.
In a review of the literature (4) we were able to find 21 cases of this association. The infecting organism was identified in 15, and in the remainder clinical features, surgical findings, or autopsy permitted a diagnosis. We were surprised to find that the mitral valve was involved in 11 of the 18 cases in which the underlying cardiac lesion could be identified; in 5 patients
