With the advent of major advances in physiological monitoring and in methods for respiratory and cardiovascular support, the management of acute severe respiratory insufficiency has become a highly sophisticated clinical undertaking. Predictably, collection of this knowledge into a readily usuable form has not kept pace with the advances. The authors have therefore addressed themselves to a timely need. The goal was to reach a wide audience, including both physicians and allied personnel. The pitfalls inherent in this approach are substantial and have been only moderately well overcome; thus, the discussions tend to assume too much background for most people in
