To the editor: Most book reviewers feel their cup is full if anyone reads their review. When an author actually accepts a reviewer's suggestion, their cup runneth over. I have experienced this good fortune.
In the November issue (Ann Intern Med 87:639-640, 1977) I praised Health Care. An International Study as a monumental advance in health services research, but I feared the book itself was of such monumental proportions that it would inhibit most readers. I suggested that a small, summary monograph was badly needed and—mirabile dictu!—it has appeared (1). In seven brief chapters and 115 pages, this little book
