To the editor: Fauci (1) in his editorial discussing the reports of Kaposi's sarcoma and opportunistic infections in young homosexual men asked two questions that I am sure have been in many readers' minds: Why homosexual men and why occurrence or recognition only as recently as 1979? The development of multiple viral bacterial and parasitic infections, or the use of illicit drugs, or both, may result in profound immune incompetence. However, many of these factors have existed in the homosexual population for years. The clustering of cases in New York City, Los Angeles, and San Francisco suggests a local factor.
