To the editor: I have followed with interest the increasing journal articles, book chapters, and books on the use of cyclophosphamide in the treatment of patients with nonmalignant diseases. Dr. Fauci has been a major contributor of articles urging the use of cyclophosphamide to treat these conditions on the basis of studies in series of patients. Such studies do not involve the comparison of cyclophosphamide treatment with alternative therapies; they do not even approach anything resembling a randomized trial.
