In the last 5 years, there has been a great resurgence of enthusiasm for using intensive insulin therapy to better control the metabolic abnormalities of diabetes. Research in this area has been intense, with many reports in many journals. At an excellent time, four distinguished investigators have now reviewed the clinical and experimental progress to date.
Diabetes mellitus is a very common, heterogeneous disease in which long-term management depends largely on the intelligence, motivation, and compliance of patients. Therefore, the authors are understandably reluctant to present intensive insulin therapy as something radically different from and clearly superior to conventional treatments.
