A book attempting to describe a rapidly changing area in medicine, for which important clinical and basic science insights occur frequently and involve several disciplines, assumes a dual challenge and responsibility. Sufficient background must be provided to allow the internist to grasp basic tenets characteristic of the field, enabling one to effectively evaluate the promises and limitations of currently available diagnostic procedures and therapeutic strategems. The book must also strive to remain timely by anticipating potential novel areas of productive research, or offering a foundation for the understanding of new developments based on analogies with other more well delineated disorders
