To the editor: Many clinicians and patients will benefit from Dr. Troupin's lucid guidelines on when to measure anticonvulsant drug plasma concentrations (1). Although pharmacokineticists have detailed many characteristics of the anticonvulsant drugs, the mere availability of such measurements without appropriate interpretation has not led to an improvement in either seizure control or avoidance of side effects (2). As Dr. Troupin and others have stated, the treatment of epilepsy is a clinical issue and not only a laboratory matter.
In discussing the therapeutic range, Dr. Troupin does not mention the specific alterations that occur in the desired therapeutic range of
