To the editor: The article by Dooley and coworkers (1) is an important discussion of the comparative diagnostic value of upper gastrointestinal radiography and endoscopy. I will not enter into the dispute concerning which is the better study (2), but Dooley and coworkers make one statement that cannot remain unchallenged. After pointing out that "In the United States, endoscopy is two to three times more expensive than the upper gastrointestinal series . . ." they state ". . . it appears that in a system in which there is no cost differential between endoscopy and the barium study (such as
