Lipoatrophy occurs with high frequency in diabetic patients treated with conventional bovine-porcine insulin preparations (1-3), but is rare in patients receiving the highly purified porcine insulins (1), although single cases have been reported (4, 5). In view of the smaller incidence of this complication with the purified insulins, insulin-induced lipoatrophy is believed to have an immune pathogenesis (6).
We report the cases of five patients with diabetes mellitus who developed lipoatrophy at the sites of insulin injection. Three of the patients were later treated with highly purified porcine insulin (Nordisk, Bethesda, Maryland), which failed to produce any regression of the
