To the editor: Nathan and associates (1) have recently investigated the effect of exercise on plasma glucose levels in fasting diabetics who were maintained on either continuous subcutaneous insulin infusion or multiple daily injections. Their data show that glucose levels remained nearly constant during exercise and fell only after patients returned to the resting state. Therefore, they recommended that exercise-induced hypoglycemia could best be prevented by pre-exercise snacks consisting of slowly absorbed carbohydrates.
I am concerned that in their effort to define experimental conditions precisely and ensure reproducibility of results, the authors may have produced recommendations that are not applicable
