To the Editor: Povar and Moreno's article (1)on health maintenance organizations (HMOs) was sheer misrepresentation, hyperbole, and bias. This article would have been justified only if it were an editorial. I fail to see how this article passed critical review.
The article misrepresented reality by defending HMOs on the basis of the "staff model," which, in reality, is the model for only a minority of operating HMOs. By their own admission, the authors state that the "independent practice model" is much harder to defend and is fraught with ethical pitfalls. Yet, this model is by far the most prevalent one
