To the Editors: Sayegh and colleagues (1) described the induction of allotolerance to renal allografts in two patients who had previously received bone marrow transplants from the same donors. We report what we believe to be the first case of a patient receiving an HLA-matched bone marrow transplant after a solid organ transplant (renal allograft) from the same donor. The post-transplant course, which was characterized by treatment-refractory, lethal graft-versus-host disease, suggests that host immune status (in this case, influenced by chronic antigenic stimulation from the previous allograft) played an important role in the pathogenesis of graft-versus-host disease.
