To the Editors: As a rheumatologist, I recently evaluated a patient with osteogenesis imperfecta, who had come for a second opinion. His enlarged bossed skull, blue sclera, and severely underdeveloped lower extremities were disturbingly distracting, even to a physician accustomed to dealing with physical deformities and who had seen such patients before.
As I take a history, I usually scan the patient's visible joints, looking for signs of inflammation or deformity. With this patient, however, it was not until I was near the end of the physical examination when I felt his warm, swollen elbows, wrists, and metacarpals and noticed
