To the editor: In the August issue, Hricik (1) suggests that volume status is an important factor in captopril-induced functional renal failure that occurs when perfusion pressure to the total renal mass is greatly reduced. Thus, in a patient with renal-artery stenosis after transplantation, Hricik has shown that after 25 mg of captropril there was a 49% reduction in inulin clearance when the patient was sodium depleted, and only a 14% reduction in inulin clearance when the patient was sodium repleted. However, it is hard to understand the marked rise in serum creatinine level that was noted the same day
