One last arterial blood gas for a patient with possible obesity hypoventilation syndrome and I would be done for the day. I entered after a quick knock, not waiting for a reply. “I need to do a blood test,” I began. She took her eyes off the TV and looked in my direction. I knew that look—the one where the patient knows you are just an intern and can be shooed away. She told me to come back later. She wanted to finish eating a bag of candy.
I was annoyed at having to waste my time on someone who clearly did not care about her health. I walked out. Then it hit me: It's not her; it's me! She doesn't care because she doesn't know. I do know, and yet I am standing outside her door waiting for her to finish a bag of candy.
